Here is a full size sample of what the front and back of the card looks like.

As you see on the front is a club logo (you obviously use your own), you name, the name of the club you are a member of and the card expiry date.

Below this is the ID Verify details. Although this is a sample card, you can check the demo by clicking here and entering the ID number on this card to see how the details would appear. You can even do this via text message too but please note charges do apply.

On the reverse side of the card as you can see we have Caduceus symbol which is a medical symbol meaning what you see here can be used in case of emergency.

We also have the card holders name, date of birth, emergency contact name and 2 telephone numbers for them. Then we have the key medical history and known allergies as advised by the Air Ambulance as key information in case of emergency.
